What is the required format for the official recording (sijill)?

The format of the sijill is: 'Peace and blessings be upon the Prophet (PBUH). This is what Judge [Name], son of [Name], Judge of the Commander of the Faithful, testified to concerning such and such, in the sitting of his judgment and jurisdiction, in such and such a location, at such and such a time, that it was established before him by the testimony of [Witness 1 Name] and [Witness 2 Name] and their lineages, whom he knew in a manner that permitted accepting their testimony. [This record is based on] what is in the copied book, and the book should be copied if it was present, or the mahzar, regardless of which ruling it concerned. When finished, it should state subsequently: 'So he ruled upon it, enforced it, and upheld it, after [Claimant Name] requested of him that he rule in his favor.'
