What is the prescribed formula and sequence for the wife's declaration during Li'an?

The judge instructs the wife to stand and say four times: 'I testify by Allah that this husband of mine is among the liars in what he has accused me with, regarding adultery.' She points to him if present, or names and identifies him if absent. After repeating this four times, she is stopped, advised similarly to the husband, and a woman is instructed to place her hand over her mouth to prevent premature declaration. If she proceeds, she is told to say: 'And the wrath of Allah be upon me if my husband is among the truthful in what he has accused me with, regarding adultery.'
