What is the prescribed manner for the call to prayer (Adhan) and Iqamah when combining two prayers in the time of the first prayer?

When combining two prayers in the time of the first prayer, it is recommended (mustahabb) to perform the Adhan for the first prayer and its Iqamah, followed by the Iqamah for the second prayer. The evidence for combining prayers at 'Arafah (Dhuhr and Asr) and Muzdalifah (Maghrib and Isha) is the narration of Jabir, stating that the Prophet, peace be upon him, combined them with one Adhan and two Iqamahs, as narrated by Muslim. Furthermore, since the first prayer is being performed in its designated time, the Adhan is legislated for it just as it would be if the prayers were not combined.
