Istijmar is permissible for unusual discharges just as it is permissible for habitual ones. The evidence is general for all discharges, and the need for Istijmar in unusual cases arises when dampness accompanies the discharge, mirroring the established condition for habitual discharges. Since avoiding it may be difficult, the assumed difficulty is considered, similar to allowing Istijmar near a flowing river. Madhi is frequently occurring for many people, sometimes more than urine, as indicated by narrations concerning Ali ibn Abi Talib and Sahl ibn Hunaif.

Some Shafi'i scholars argue that Istijmar is not permissible for Nadher. Ibn 'Abd al-Barr suggests this may align with Malik's view because the Prophet (PBUH) commanded washing the penis specifically for Madhi, implying obligation. They argue that all reliable reports only mention washing, not Istijmar for Nadher, and since Nadher is not recurrent, the relevance of water is diminished, requiring washing as with other areas. Regarding Madhi, one narration states washing the penis and testicles is obligatory out of obedience, while another states the command to wash is for recommendation only, by analogy with other excretions.
