What declaration must be made during the slaughter of the sacrificial animal?

The slaughterer must say, 'In the name of God, and God is the Greatest' (Bismillah, Wallahu Akbar). This practice is attested by the actions of the Prophet (peace be upon him) and Ibn Umar. There is no known disagreement regarding the recommendation to utter this phrase or the sufficiency of the Tasmiyah (uttering the name of God). If the Tasmiyah is forgotten, the slaughter is still valid, following the general rule for permissible slaughter.
Supporting text
It is commendable to add the supplication: 'O God, this is from You and for You. O God, accept it from me, or from so-and-so.' Most scholars hold this to be good.
