Musannaf Ibn Abi Shayba 2:188

Ibn Mahdi narrated to us, from Hammad ibn Salamah, from Hisham ibn Urwah, that a man came to the mosque and they had already prayed. He went to perform the iqamah, so Urwah said to him: "Refrain, for we have already performed the iqamah."
