Musannaf Ibn Abi Shayba 16:40

Abu Bakr narrated to us, he said: Yazid ibn Harun narrated to us, he said: Hisham al-Dastawa'i informed us, from Qatadah, he said: "One should pronounce the name (of Allah) over the 'aqiqah just as one pronounces the name (of Allah) over the sacrificial animal (udhiyah): 'In the name of Allah, the 'aqiqah of so-and-so.'"
