Musannaf Ibn Abi Shayba 33:1259

Abu Usama narrated to us, from Wuhayb, from Musa ibn Uqba, who said: I heard Ibn Muhayriz while we were with him in Ramla, and he was saying: "I reached the people, and when a deceased person among them from the Muslims died, they would say: 'Praise be to Allah who caused so-and-so to die upon Islam.' Then that ceased, and there is no one today who says that."
