Musannaf Ibn Abi Shayba 33:31

Yazid ibn Harun narrated to us, he said: Mubarak informed us, from al-Hasan, he said: The Prophet Dawud (peace be upon him) used to say: "O Allah, [grant me] neither illness that weakens me, nor health that makes me forget, but [something] between those two."
