Musannaf Ibn Abi Shayba 9:141

Al-Muharibi narrated to us, from Layth, from al-Hakam, who said: The companions of the Messenger of Allah, may Allah bless him and grant him peace, reached a consensus that a slave is not to be married to more than two women.
