Musannaf Ibn Abi Shayba 3:1272

Waki’ narrated to us, from Shu’bah, from Hammad, from Ibrahim, who said: If he recites aloud in that which should be recited silently, or recites silently in that which should be recited aloud, then upon him are the two prostrations of forgetfulness.
