Musannaf Ibn Abi Shayba 3:1777

Waki’ narrated to us, from Shu’bah, from al-Azraq ibn Qays, who said: I heard Ibn al-Zubayr recite "In the name of Allah, the Most Gracious, the Most Merciful." Then he recited "All praise is due to Allah, Lord of the worlds." Then he recited "In the name of Allah, the Most Gracious, the Most Merciful."
