Musannaf Ibn Abi Shayba 3:1783

Waki’ narrated to us, from Sufyan, from ‘Asim, from Sa’id ibn Jubayr, that he used to recite "In the name of Allah, the Entirely Merciful, the Especially Merciful" in every rak'ah.
