Musannaf Ibn Abi Shayba 3:2105

Waki’ narrated to us, from Sufyan, from al-Hasan ibn ‘Ubayd Allah, from Ibrahim; and from Shu’bah, from Jabir, from al-Sha’bi: They both said: In every forgetfulness, there are two prostrations.
