Musannaf Ibn Abi Shayba 3:2107

Waki' narrated to us, from Sufyan, from Abu Farwah, who said: Abd al-Rahman ibn Abi Layla led us in prayer. When he sat [for the final tashahhud] and moved to stand up, he performed the two prostrations of forgetfulness (sajdatay al-sahw).
