Musannaf Ibn Abi Shayba 3:2174

Waki’ narrated to us, from Hushaym, from Mughira, from Ibrahim, who said: He used to say: If forgetfulness (sahw) enters upon the Imam and he does not know what it is, then he should signal to those who are behind him.
