Musannaf Ibn Abi Shayba 3:91

Isbat ibn Muhammad narrated to us, from Mutarrif, from Hammad, who said: "If the Imam forgets the first Takbir with which he opens the prayer, he must repeat [the prayer]." And al-Hakam said: "The Takbir of bowing suffices him."
