Musannaf Ibn Abi Shayba 21:23

Jarir narrated to us, from Mansur, from Habib ibn Abi Thabit, who said: It was narrated to me that the Prophet, may Allah bless him and grant him peace, used to say: "O Allah, I seek refuge in You from a supplication that is not heard, and knowledge that does not benefit, and a heart that does not humble itself, and a soul that is not satisfied. O Allah, I seek refuge in You from the evil of these four. O Allah, I ask You for a balanced life, a pure death, and a return to You that is not disgraceful."
