Musannaf Ibn Abi Shayba 21:237

Muhammad ibn Bishr narrated to us, he said: Ishaq ibn Rashid narrated to us, from Abdullah ibn al-Hasan: That Abdullah ibn Ja’far entered upon a son of his who was ill, who was called Salih. He said to him: Say, "There is no god but Allah, the Forbearing, the Generous. Glory be to the Lord of the Great Throne. Praise be to Allah, Lord of the worlds. O Allah, forgive me. O Allah, have mercy upon me. O Allah, overlook my faults. O Allah, pardon me, for You are the Pardoning, the Forgiving." Then he said: My uncle taught me these words, mentioning that the Prophet, may Allah bless him and grant him peace, taught them to him.
