Musannaf Ibn Abi Shayba 21:25

Al-Fadl ibn Dukayn narrated to us, from Sa’d ibn Aws, from Bilal ibn Yahya, who said: Shutayr ibn Shakl narrated to me, from his father, who said: I came to the Prophet, may Allah bless him and grant him peace, and said: "O Prophet of Allah, teach me a supplication for refuge by which I may seek refuge." He said: "Say: O Allah, I seek refuge in You from the evil of my hearing, my sight, my tongue, and my semen."
