Musannaf Ibn Abi Shayba 21:274

Al-Hasan ibn Musa narrated to us, Hammad ibn Salamah narrated to us, from Sa'id al-Jurayri, from Abu al-'Ala', from Uthman ibn Abi al-'As and a woman from Qays, that they both heard the Prophet, may Allah bless him and grant him peace. One of them said: I heard him saying, "O Allah, forgive my sin and my mistakes and my intentional acts." The other said: I heard him saying, "O Allah, I ask You for guidance to the right course of my affair, and I seek refuge in You from the evil of my soul."
