Musannaf Ibn Abi Shayba 21:37

Ali bin Hashim narrated to us, from Ibn Abi Layla, from Ishaq al-Jazari, from Abu Ja'far, who said: The words of relief are: "There is no god but Allah, the Most High, the Magnificent. Glory be to Allah, Lord of the Noble Throne. Praise be to Allah, Lord of the worlds. O Allah, forgive me, have mercy upon me, overlook my faults, and pardon me, for You are the Forgiving, the Merciful."
