Musnad Abu Ya'la al-Mawsili 152:199

Muhammad ibn ‘Abbad al-Makki narrated to us, Abu Sa‘id, the freed slave of Banu Hashim, narrated to us, he said: al-Mas‘udi narrated to us, from ‘Awn, from Abu Fakhitah, from al-Aswad, from ‘Abdullah, who said: "When you pray upon the Messenger of Allah (may Allah bless him and grant him peace), then make the prayer upon him excellent, for you do not know, perhaps that is presented to him." They said: "Then teach us, O Abu ‘Abd al-Rahman." He said: "Say: O Allah, place Your prayers, Your mercy, and Your blessings upon the master of the messengers, the leader of the God-fearing, and the seal of the prophets, Muhammad, Your servant and Your messenger, the leader of goodness, the guide to goodness, and the messenger of mercy. O Allah, raise him to a praised station which the first and the last will envy him for. O Allah, pray upon Muhammad and upon the family of Muhammad, as You prayed upon Ibrahim and upon the family of Ibrahim; indeed, You are Praiseworthy, Glorious. And bless Muhammad and the family of Muhammad, as You blessed Ibrahim and the family of Ibrahim; indeed, You are Praiseworthy, Glorious."
