Musnad Abu Ya'la al-Mawsili 252:26

And from Abu Hazim, he said: Sahl ibn Sa'd informed me, he said: I witnessed three things from the Messenger of Allah, may Allah bless him and grant him peace, when his incisor tooth was broken, his face was wounded, and the helmet was shattered upon his head. And I truly know who was washing the blood from his face, and who was carrying the water for him, and what was placed for the one who took it.
