Musnad Ahmad 25:330

I read to Abd al-Rahman, from Malik, from Abu al-Zubayr al-Makki, from Tawus al-Yamani, from Abdullah ibn Abbas: That the Messenger of Allah, peace and blessings of Allah be upon him, used to teach them the supplication just as he used to teach them the Surah from the Qur’an. He would say: "Say: O Allah, I seek refuge in You from the punishment of Hell, and I seek refuge in You from the punishment of the grave, and I seek refuge in You from the trial of the False Messiah (al-Masih al-Dajjal), and I seek refuge in You from the trial of life and death."
