Musnad Ahmad 26:541

Yahya narrated to us, from al-A’mash, and Waki’ narrated to us, al-A’mash narrated to us, he said: Zayd ibn Wahb narrated to us, from Abdullah, he said: The Messenger of Allah (peace and blessings of Allah be upon him) narrated to us, and he is the truthful, the believed: "Indeed, the creation of one of you is gathered in the womb of his mother for forty days—or he said: forty nights, Waki’ said: nights—then he becomes a clinging clot for the like of that, then he becomes a chewed lump for the like of that. Then Allah, the Almighty and Majestic, sends to him the angel with four words: his deed, his lifespan, his provision, and whether he is wretched or happy. Then the soul is breathed into him. By the One besides Whom there is no god, indeed one of you performs the deeds of the people of Paradise until there is nothing between him and it but a cubit, then the decree overtakes him, and he is sealed with the deeds of the people of the Fire, so he becomes one of them. And indeed one of you performs the deeds of the people of the Fire until there is nothing between him and it but a cubit, then the decree overtakes him, and he is sealed with the deeds of the people of Paradise, so he becomes one of them."
