Musnad Ahmad 830:37

Waki' narrated to us, Uthman al-Shahham narrated to us, Muslim ibn Abi Bakrah narrated to us, from his father, that the Prophet, peace and blessings of Allah be upon him, used to say at the end of every prayer: "O Allah, I seek refuge in You from disbelief, poverty, and the punishment of the grave."
