Musnad Ahmad 1007:103

'Affan narrated to us, Hammam narrated to us, Yahya ibn Abi Kathir narrated to us, 'Abdullah ibn Abi Qatadah narrated to us, from his father, that he witnessed the Prophet, peace and blessings of Allah be upon him, praying over a deceased person, and he heard him saying: "O Allah, forgive our living and our dead, our present and our absent, our young and our old, and our male and our female." He said: And Abu Salamah ibn 'Abd al-Rahman narrated these eight phrases to me, and added two phrases: "Whoever among us You keep alive, keep him alive upon Islam, and whoever among us You cause to die, cause him to die upon faith."
