Musnad Ahmad 1156:232

'Abd al-Rahman narrated to us, from Sufyan, from Mansur, from al-Sha'bi, from Umm Salamah, that the Messenger of Allah (peace and blessings of Allah be upon him), when he would leave his house, used to say: "In Your name, my Lord, I seek refuge in You from slipping or being caused to slip, or from wronging or being wronged, or from acting ignorantly or having ignorance acted upon me."
