Musnad Ahmad 1156:257

Muhammad ibn Ja‘far narrated to us, Shu‘bah narrated to us, from Mansur, from al-Sha‘bi, from Umm Salamah, that the Messenger of Allah (peace and blessings of Allah be upon him) used to say when he left his house: "In the name of Allah." Shu‘bah said: "The greatest of my knowledge is that he said it." He said: "Sufyan also mentioned it from him, and there is no doubt in the rest of it: O Allah, I seek refuge in You from going astray or being led astray, or from slipping or being made to slip, or from oppressing or being oppressed, or from acting ignorantly or having ignorance acted upon me."
