Musnad Ahmad 299:2

Rawh and ‘Abd al-Samad narrated to us, they said: Hammad narrated to us, Rawh said: Al-Jurayri informed us, from Abu al-‘Ala’, from ‘Uthman ibn Abi al-‘As and a woman from Qays, that they both heard the Prophet, may Allah bless him and grant him peace. One of them said: I heard him saying: "O Allah, forgive me my sin, my mistake, and my intentional acts." The other said: I heard him saying: "O Allah, I seek Your guidance to direct my affairs, and I seek refuge in You from the evil of my soul."
