Musnad Ahmad 572:9

Hassan ibn Musa narrated to us, Hammad ibn Salamah narrated to us, from Sa’id al-Jurayri, from Abu al-‘Ala’, from Uthman ibn Abi al-‘As and a woman from Qays, that they both heard the Prophet, may Allah bless him and grant him peace. One of them said: I heard him saying: "O Allah, forgive my sin, my mistake, and my intentional act. O Allah, I seek Your guidance to direct my affair, and I seek refuge in You from the evil of my soul."
